Causes of development of
Until now, the causes leading to the development of Croker-Williams erythema have not been identified. Not defined and the final place of this disease in the classification of skin diseases.
It is accepted to allocate the symptomatic form of erythema, which develops on the background of different pathologies. Infectious diseases, ulcerative colitis, polyarthritis, etc. can become the root cause of erythema development.
In addition, there is another form of the disease that occurs much less often - idiopathic, that is, caused by obscure reasons.
In the literature, cases of stable disease are described by family erythema, therefore it is impossible to categorically deny hereditary predisposition to this disease.
Clinical picture of
Two types of rheumatoid arthritis described in the literature are essentially the stages of one process:
- The Hutchinson type is an early period of the disease. At this stage, the process is histologically close to the vasculitis.
- Type Bari( Bury) is a later period of the disease. At this stage, the histologic pattern of lesions is more like a ring-shaped granuloma supplemented by fibrosis.
Creator-Williams erythema manifests itself by the appearance of symmetrically arranged rashes. On the skin there are numerous small papules, which gradually increase, merging into large plaques with a diameter of 5-7 sm. The contours of the hearth are incorrect, within the limits of the formations there are rollers.
The color of the centers with rhythmic erythema ranges from pink to burgundy red, the old cells can get a yellowish tinge. Therefore, in the long run, the disease of cells can be similar to the manifestations of xanthomas.
At the first stage, the plaque has a soft consistency, but over time their surface is sealed. The surface of the plaques, as a rule, is smooth, but sometimes it can be observed and peeling of the surface of erythematous foci.
The appearance of hemorrhages, bullous cells or ulcers for Croker-Williams erythema is a rare phenomenon. But when joining secondary infections, symptoms that are uncharacteristic for the classic course of the disease may occur.
Various clinical symptoms can be observed and, in the generalized form, rises with erythema. In this case, it is possible to develop a clinical picture, similar to the symptoms of exudative multiform erythema.
At the initial stage of Croker-Williams erythemates, subjective symptoms such as burning, itching, and soreness may occur. These manifestations can be aggravated under the influence of cold as in cryoglobulinemia.
Rash with eritum rises, most often, they are located on the limbs in the joints, at the feet as with nodular erythema. Rarely rashes appear on the face, even less - on the trunk. Equally rarely rash appears on the mucous membranes.
Enhanced erythema is a disease that affects mostly middle-aged people. The course of the disease is chronic, the development is slow.
At the site of the resolved lesions, areas of hyperpigmentation are formed. In places of particularly large plaques, areas of atrophy may appear, and in the case of developing ulcers, scarring may still occur.
The diagnosis of erythema is based on the study of clinical and histological picture.
Raised erythema of steady flow must be distinguished from:
- ring-shaped erythema Daryna;
- of lipoid necrobiosis;
- Dühring's Dermatitis;
- ring-shaped granulomas.
Under the typical course of ring-shaped erythema, there are no papillary elements. The disease appears with the formation of fast growing spots, turning into ring-shaped elements. Rashes with erythema Daryni are regressed much faster than those found in Crokeer-Williams's steady erythema.
Differentiate rises to erythema with sarcoidosis if rash is formed on the face. The main difference of sarcoidosis is the isolation of the elements, their cyanotic color, as well as the involvement in the process of internal organs.
The manifestations of persistent erythema are largely similar to the symptoms of lipid necrobiosis, therefore histological examination is required to clarify the diagnosis.
The histologic picture, when rendering erythema, depends on the stage of the process. At the initial stage of the disease, there are signs that are characteristic of an allergic vasculitis. In the dermis there are sharply enlarged vessels with thick walls, infiltrate consists of lymphocytes, neurophylls and histiocytes. Sometimes there is not enough number of eosinophils and plasma cells.
In the later stages of the disease there are phenomena of fibrosis, secondary deposits of lipids in the dermis, slightly hyperkeratosis and acanthosis.
Specific Effective Therapy for the Treatment of Sustained Erythema E is developed. The scheme used for treatment of allergic vesiculitis is used. Patients prescribe the administration of nonsteroidal anti-inflammatory drugs, low molecular weight heparins, disaggregation drugs. When erythema rises, it is mandatory to prescribe vitamin therapy, taking calcium, routine, angioprotectors.
With the appearance of large erythematous plaques, education is recommended to be removed by methods of cryotherapy. A good effect is to conduct a course of injections of corticosteroids around lesions.
Treatment of folk remedies
Exacerbation of erythema is a very serious disease, therefore, folk remedies can only be used as an adjunct to basic therapy.
Treatment of Croker-Williams erythema may be helpful in the medicinal plant arnica. From the flowers of this plant you can prepare a healing infusion. It is necessary to brew in a thermos crushed flower arnica in a ratio of 1 spoon of raw material to a glass of water. In the morning received an infusion of strain and drink 1-2 sips every 1.5-2 hours.
Roots of arnica can be used for ointment. To prepare the preparation, you will need 100 grams of dried root of arnica and the same amount of fresh spiced pork fat. You need to rub the roots into powder, mix with grease and put in a closed clay pot in the oven. Tomita at a temperature of 120 degrees for 3 hours. The loan is allowed to cool down without getting out of the oven. Use the resulting ointment twice a day to apply to the area of rash with stable erythema.
Prevention and prognosis of
Specific prophylaxis is not developed due to erythema, because the causes that cause the disease are unclear. To prevent the development of the symptomatic form of the disease, regular medical examinations should be carried out and, if any diseases are detected, to treat them in a timely manner.
The prognosis of unstable erythema surviving, the disease proceeds long, often recurring.
